Fixed possible progression-break in Upper Sewers. Added a fix for saves affected by previous issue. Upper Sewers' boss should now spawn properly. Some players who have manually edited their save to add the Silversword may be experiencing the same progression break and progression lock. They will need to manually remove the Silversword for the progression-fix to work (and to possibly prevent later progression breaks).
